The interview I had today was very interesting. I met with a different person and she was very nice. She told me she was intrigued by my answers from my first interview (she was not part of that one). She told me that I interview very well and was very interested in me. The position she is looking to fill is for an appeals examiner. She also told me that eventually this position would be doing some arbitration and there would be training. She also asked if that interested me and of course it intrigues me. To me it sounded like what a legal nurse consultant would do.

At the end of the interview she again told me "I am very interested in you, but that is as far as I can go at the moment". They are waiting for the funding to come through for the contract which would be a five year contract.

I am really excited about this opportunity! I hope they get the funding soon!!!!!

Negotiations went fine, they quickly agreed to the the amount I suggested. Which makes me think that I probably should have asked for more. Oh well, at least I'm not taking a pay cut.

I did wish I had some time off, but they're eager to get me started ASAP so I really didn't think I could negotiate a week off. Going to see what I can do with my present schedule...

Also might have to look into COBRA to cover the period before my new insurance kicks in, though it'd sure be expensive...

But, most importantly, I got the job, and in two weeks I'm going to be free of this place and of bedside nursing! th_bf-swinging-00Heeyyyy Smileyth_wootth_Laie_60B1

Nurses' duties in 1887

[The item below is from a newspaper clipping I found in my mother's old Bible. CareNurse is honored that it was selected from our website to be published in a 2005 Elsvier/Mosby nursing text, Contemporary Nursing.]
The following job description was given to floor nurses by a hospital in 1887:
In addition to caring for your 50 patients, each nurse will follow these regulations:

Daily sweep and mop the floors of your ward, dust the patient's furniture and window sills.

Maintain an even temperature in your ward by bringing in a scuttle of coal for the day's business.

Light is important to observe the patient's condition. Therefore, each day fill kerosene lamps, clean chimneys and trim wicks. Wash the windows once a week.

The nurse's notes are important in aiding the physician's work. Make your pens carefully; you may whittle nibs to your individual taste.

Each nurse on day duty will report every day at 7 a.m. and leave at 8 p.m. except on the Sabbath on which day you will be off from 12 noon to 2 p.m.

Graduate nurses in good standing with the director of nurses will be given an evening off each week for courting purposes or two evenings a week if you go regularly to church.

Each nurse should lay aside from each pay day a goodly sum of her earnings for her benefits during her declining years so that she will not become a burden. For example, if you earn $30 a month you should set aside $15.

Any nurse who smokes, uses liquor in any form, gets her hair done at a beauty shop, or frequents dance halls will give the director of nurses good reason to suspect her worth, intentions and integrity.

The nurse who performs her labors and serves her patients and doctors without fault for five years will be given an increase of five cents a day, providing there are no hospital debts outstanding.

Several days ago as I left a meeting at a hotel; I desperately gave myself a personal TSA pat down. I was looking for my keys. They were not in my pockets. A quick search in the meeting room revealed nothing.

Suddenly I realized I must have left them in the car. Frantically, I headed for the parking lot. My wife has scolded me many times for leaving the keys in the ignition.
My theory is the ignition is the best place not to lose them.
Her theory is that the car will be stolen. As I burst through the door, I came to a terrifying conclusion.

Her theory was right. The parking lot was empty.

I immediately called the police. I gave them my location, confessed that I had left my keys in the car, and that it had been stolen.
Then I made the most difficult call of all, "Honey," I stammered; I always call her "honey" in times like these.
"I left my keys in the car, and it has been stolen."

There was a period of silence. I thought the call had been dropped, but then I heard her voice.
"Idiot", she barked, "I dropped you off!"

Now it was my time to be silent. Embarrassed, I said, "Well, come and get me."

She retorted, "I will, as soon as I convince this policeman I have not stolen your car."

Yep it's the golden years. th_laughs9

Went fairly well, I thought, though there's no getting around the fact that I have no case management experience.

Though actually it's more care coordination than case management, which is good as the caseload is pretty high. For the fairly intensive Medicaid patients, with multiple hospitalizations, it's up to 50 cases. If it's an average Medicaid patient with no real issues and minimal use of the system, it's up to 250. Though they say the average is only half that.

There is a backup oncall rotation, a week long, but I think it's only twice a year and is rarely utilized. The case mangers I spoke to said they almost always leave on time and when late it's less than an hour. There is some travel to visit patients but it's only a few times a year.

So far so good, I haven't seen any obvious red flags. The people I met seem pleasant and laid-back. We'll see if anything comes of it...

They didn't ask me a lot of questions, they said the resume pretty much said it all.

This HH agency is actually providing staffing for a major insurance company you've all heard of. The position is case management for a new program that manages some of the state's Medicaid patients. I believe it's called an "integrated care model". Work downtown, between 20 and 50 cases, usual business hours. The insurance company would provide the training and benefits, though I'd technically be employed by the HH agency. Not completely clear whether this is a contract position or a full-time position, but if they offer it to me I'll be sure to ask before I accept.

They want it filled before the end of the month. So far it sounds good.

Been having to deal with this once again...all because when asked when I wanted to go on a break I said honestly what would work for me...got so badly bashed I didn't want to socialize all night (and didn't).

Well tonight one of the same bullies was working in the same room with me...and of coarse turned it all around now acting as though she has been abused!? OMG!!! Such immaturity!!!

So far tonight at least I have not stepped down to her level....but Come ON!!! Seriously!!??
Thing is she is in the work clique and I'm quite certian I'll be bashed several times without being able to defend myself!

Following a urinary-retention surgery, Charles E. Shreve was allegedly informed by a nurse at the Tennessee Urology Association that he would have to perform a self-catheterization at home, for “continuous bladder irrigation.”

According to the lawsuit filed by Shreve and his wife, Shreve was told to insert a catheter through his penis into his urethra. The complaint says that Shreve, who had no prior experience in self-catheterization, and was not directed by medical staff regarding the proper way to self-catheterize,
got the catheter about three inches in when he experienced severe pain, and allegedly lost consciousness, hit his mouth, knocked out three teeth, and broke his neck. The suit claims that when Shreve woke, he was paralyzed from the neck down.

The lawsuit claims that his nurse made the decision for Shreve to self-catheterize on her own, and without consulting with his physician. The complaint also says that Shreve was given no instruction regarding how to perform the self-catheterization and that while he was attempting to do it
the nurse had her back to him and “not in a position to prevent or guard against his fall.” Shreve and his wife have filed a lawsuit against his physician, John-Paul Lamar Newport, and the Tennessee Urology Association seeking $10 million in damages.

So I just got home from clinical. My group is in our "pediatric" rotation. I put that in quotes because we are in a long term pedi facility that has a population of kids from ages 5-late 40's. a few younger (a 5yo, 6yo, couple each of 16,17,18,and19 yo's), all the rest are late 20's - 40's. Today I had a man who was 39. Anoxic brain injury from birth had been in here since he was 6 months old. I was told by someone who works on the floor when I walked in his room I should say "I hear Timmy gives kisses". He blew kisses to me when I did.

The CNA's needed to do the transfer with the Hoyer (facility policy that students cant). Both of them asked for kisses and he would not (kept mouth tightly closed and looked the other way). After they left I asked for kisses and got some. We were paired up today and my partner and I were getting him ready for bed. We were having trouble getting the danged diaper on him (always have trouble positioning it right) and he was laughing at us. I said to him "Timmy are you laughing at us?" and he started laughing harder. One person said the laughing was just coincidence but everyone else said he totally knows what is going on around him, just can't talk. One person figured he was thinking "Well I know what you should be doing why dont you?"

I just had an amazing night and enjoyed it so much and wanted to share it.
